Discovery Phase: Uncovering the Full Picture of Your Health

The Discovery Phase is designed to gather the deeper insights needed to create a truly personalized path forward. During this phase, Dr. Danielle Wojtak will carefully review your previous laboratory results, imaging, medical records, and experiences with other healthcare providers to better understand your unique health journey.

This is also a time to complete any recommended labs, imaging, or additional assessments identified during your initial visit. By combining your health history with objective data, Dr. Wojtak works to identify patterns, uncover opportunities for support, and develop a clear understanding of the foundations needed for your individualized care plan.

The Discovery Phase lays the groundwork for a thoughtful, root-cause approach — ensuring your next steps are guided by a complete view of your health.

Your Personalized Care Plan brings together the insights gathered throughout your Initial Visit and Discovery Phase to create a thoughtful, individualized approach to your health. Drawing on Dr. Danielle Wojtak’s expertise in naturopathic medicine, clinical experience, and understanding of your unique health patterns, she will develop recommendations designed to support your goals and the foundations of lasting wellness.

Because you know your body better than anyone, your voice and experience are an essential part of the process. At Root & Reason Naturopathic Medicine, we believe that each person knows their body better than anyone else. Our job is to collaborate with you to create a treatment plan that gets you to your health goals. We work through each visit from a collaborative care model- patients and doctors working together to build a treatment plan.

We are currently a telehealth only practice- scheduling phone calls or video visits. We work with patients locally in Bozeman, MT as well as virtually in the rest of Montana and Wisconsin. If you are currently living outside of Montana and Wisconsin, we work with you as a health consultant, which means we cannot diagnose and you must have a Primary Care Provider locally to help us coordinate care- ordering labs and imaging as needed.
